From the Author Anxiety disorders are more common in Australia than depression; roughly 14.4% of the population suffer from an anxiety disorder, and the disability caused by these disorders is comparable to that caused by physical disorders. Yet less than half of the people who need it get adequate treatment. Over half seek treatment by visiting a doctor but only 60% of those will receive treatment that is known to be beneficial. The rest receive counselling or inadequate advice. The aim of this handbook is to provide a solid understanding of the biological and psychological theories about the anxiety disorders and the epidemiology and clinical features of various anxiety disorders. It also aims to provide the basis for psychological and pharmacological treatments. The handbook sets out the theoretical models as to why anxiety disorders arise, and show that there is no single cause of an anxiety disorder. Rather, they are best understood through a biopsychosocial model, in which there are biological, psychological and social explanations for their origins. Along with psychological theories about anxiety disorders, the physiology of anxiety and the roles of the autonomic nervous system are described, as is that of the hypothalamic-pituitary-adrenal axis. The brain networks involved in anxiety - the fear circuitry - are explained, as well as genetic theories that may underlie a propensity to anxiety. From a psychological perspective, learning theory and the different forms of conditioning are described. But there are also other important psychological theories about anxiety, including disturbances in attachment that underpin many of the psychodynamic theories of anxiety disorders. The way people appraise or think about events has an enormous impact on their response to those events, and this is important to understand in order to comprehend the cognitive theories of anxiety disorders. Anxiety disorders do not occur in a vacuum, and there are many aspects of the social environment that contribute to their onset. These include the life events that may trigger anxiety, while social support may help buffer against the impact of such stresses. In understanding anxiety disorders from a biopsychosocial perspective, it is crucial to realise that these different domains interact with each other in contributing to the onset and continuation of an anxiety disorder. It is important to confirm the type of anxiety disorder from which the individual is suffering, to find out about the context of the disorder, what triggered it, what maintains it and why it has developed. It is also important to ensure, as far as possible, that anxiety is not due to an underlying medical illness. Given that there are different mechanisms involved in the development of anxiety disorders, it is hardly surprising that there are different approaches to treating anxiety disorders. These approaches include psychological treatments and pharmacological treatments. There is now a range of evidence-based treatments for the anxiety disorders from a psychological perspective, such as the exposure-based behavioural treatments and cognitive treatments which will challenge cognitive distortions and assist patients to manage worry and rumination, which can be conducted face-to-face or over the Internet. There are also well-established pharmacological treatments for anxiety disorders; a range of medications that have demonstrated effectiveness in the treatment of the disorders, in particular the serotonin reuptake inhibitors, which are widely used in their treatment, and the benzodiazepines. This handbook draws on the expertise of specialists from around Australia and New Zealand, who have combined their knowledge to create a comprehensive guide to all aspects of anxiety disorders and their treatment.

The text is supplemented with periodic case vignettes and tables and that provide a practical and interesting adjunct to the theoretical information. The book emphasizes the importance of understanding the interplay between the biopsychosocial domains in contributing to the onset of anxiety and perpetuation of the problem.The overall tone of the text is designed to educate the reader.
